New Patient Appointments

What to Bring: Please bring your Saskatchewan Health Card, a list of all current medications, any relevant past medical records, blood pressure or blood sugar logs and a list of questions you may have.

Follow-Up Appointments

What to Bring: Please bring your Health Card and an updated list of your medications (your other doctors may make changes to your medication list that we are not aware of). You may also consider calling your pharmacy and asking them to fax us a list of your current medications (fax number 306-565-39290.

Frequency: The frequency of follow-up appointments will be determined by your specific medical condition and treatment plan.

Referral Information

Dr Lwanga only sees patients on a referral basis. Please ensure your physician or nurse practitioner sends a referral prior to your first appointment.

  • During your appointment Dr. Lwanga and the team will work with you to review your medical history and develop a personalized care plan.

  • Based on your needs, we may schedule follow-up appointments to monitor your progress, adjust medications, provide ongoing education, and ensure your treatment plan remains effective.

  • We believe in a team approach. We will communicate regularly with your family physician to ensure seamless and coordinated care.
